Life! type 2 diabetes prevention program

obeseThis year BCHC and BHS have began running the Life! program in Ballarat, which is a lifestyle modification program aimed at preventing diabetes in people 50 and over. The Life! program is funded by Diabetes Victoria to target the estimated 500,000 Victorians who are at high risk of developing type 2 diabetes.

Can allied health professionals receive an EPC referral without contributing to TCA (Item 723)?

As you are no doubt aware, to be entitled to EPC referrals patients must be managed under both GPMP (item 721) and the TCA (item 723). Together these are referred to as Enhanced Primary Care (EPC). When developing a TCA, GPs must include 2 or more service/treatment providers with whom there is two-way communication. (more…)
