Empower your patients to take control of their health
At Ballarat Community Health Sebastopol our team run the evidence-based Better Health Self Management Course, which was designed, trialled and fine-tuned at Stanford University in the USA over the last 20 or so years.
The group runs one 2.5-hour session a week for six weeks, focussing on self-management of clients with chronic health conditions.
The sessions are very interactive, with:
- goal setting and evaluation each week
- brainstorming for participants to find solutions to the difficulties they face on a daily basis
- opportunities for clients to network and listen to others who suffer from a variety of chronic health conditions
A broad range of topics are covered, including:
- managing symptoms
- problem solving
- making actions plans
- dealing with difficult emotions, fatigue, pain
- communication
- making treatment decisions
- working with you healthcare professionals
- nutrition
- fitness
It is aimed at empowering clients to manage their own condition by teaching them the skills that will enable them to do so.
