Our HMR coordinator works with general practice, pharmacies and patients to make sure medicines are being used effectively. We can;
- provide HMR support to GPs and practices
- help source HMR accredited pharmacists
- provide HMR referral templates (see below)
- facilitate HMR accreditation for pharmacists
- coordinate HMR meetings for pharmacists and GPs
